Simon I. Polisky

Age 64, passed from this life August 21, 2010 in Arlington, Texas. Simon was born April 19, 1946 in Chicago, Illinois to parents Eddie and Florence Polisky. Simon retired from Texas state employment in December of 2008 after serving as a Long Term Care Regulatory Surveyor for 35 years. He inspected nursing homes, assisted living facilities and adult day care facilities, as well as conducted compliance investigations throughout the 19 counties of Region 3. He was officed at the Texas Department of Aging and Disability Service facility on Matlock Road in Arlington, TX. He was dedicated to assuring that individuals residing in Long Term Car in Region 3 received the very best care possible. Simon is survived by his wife, Nancy Smith Polisky, children and step children; Roger Smith and his wife Kelly, Anita Gorman and husband Charles, David Polisky and Sarah Polisky, grandchildren; Brandon and Brian Gorman and Bailey and Bethany Smith, sister, Kotch Bergman and aunt and uncle Gene and Pearl Korey.
